Employment Application Form
Conditions of employment are stated at the end of this form. Please read carefully before you sign this application. False statements on this application form shall be considered sufficient cause for termination.
Criminal Offenses Checked by Statute
The statute defines two main categories of crimes that an inquiry will focus on:
1. Crimes Against Children or Other Persons
This includes a conviction of any of the following offenses, or any of these crimes as they may be renamed:
Homicide/Assault: Aggravated murder; first or second degree murder; first or second degree manslaughter; first, second, or third degree assault; vehicular homicide; custodial assault.
Sexual Offenses: First, second, or third degree rape (including rape of a child); indecent liberties; incest; sexual exploitation of minors; first, second, or third degree child molestation; first or second degree sexual misconduct with a minor; patronizing a juvenile prostitute; prostitution; felony indecent exposure.
Kidnapping/Imprisonment: First or second degree kidnapping; unlawful imprisonment; first or second degree custodial interference.
Robbery/Burglary: First or second degree robbery; first degree burglary.
Arson/Malicious Harassment: First degree arson; malicious harassment.
Abuse/Neglect: First or second degree criminal mistreatment; child abuse or neglect (as defined in RCW 26.44.020).
Child/Prostitution-Related: First degree promoting prostitution; communication with a minor; child abandonment; promoting pornography; selling or distributing erotic material to a minor; violation of a child abuse restraining order; child buying or selling.
Other: Simple assault; first or second degree extortion.
2. Crimes Relating to Financial Exploitation
This includes a conviction for any of the following offenses, or any of these crimes as they may be renamed:
First, second, or third degree extortion.
First, second, or third degree theft.
First or second degree robbery.
Forgery.
Records Check and Notification
If you are offered a position as a paid employee or volunteer, the District may submit an inquiry to the Washington State Patrol (under RCW 43.43.832 and .834) to verify your answers.
Notification: You will be notified within ten (10) days after the District receives a response from the State Patrol.
Copy: You will be provided a copy of the response at your request.
Usage: The District will use this information only to make the initial employment decision.
A crime against children or other persons is defined by the statute as:
“…a conviction of any of the following offenses: Aggravated murder; first or second degree murder; first or
second degree kidnapping; first, second, or third degree assault; first, second, or third degree rape; first, second,
or third degree rape of a child; first or second degree robbery; first degree arson; first degree burglary; first or
second degree manslaughter; first or second degree extortion; indecent liberties; incest; vehicular homicide; first
degree promoting prostitution; communication with a minor; unlawful imprisonment; simple assault; sexual
exploitation of minors; first or second degree criminal mistreatment; child abuse or neglect as defined in RCW
26.44.020; first or second degree custodial interference; malicious harassment; first, second, or third degree child
molestation; first or second degree sexual misconduct with a minor; first or second degree rape of a child;
patronizing a juvenile prostitute; child abandonment; promoting pornography; selling or distributing erotic material
to a minor; custodial assault; violation of child abuse restraining order; child buying or selling; prostitution; felony
indecent exposure; or any of these crimes as they may be renamed in the future”.
A crime relating to financial exploitation is defined by statute as:
“…conviction for first, second, or third degree extortion; first, second, or third degree theft; first or second
degree robbery; forgery; or any of these crimes as they may be renamed in the future.”
If you are offered a position as a paid employee or volunteer with the District, the District may under RCW
43.43.832 and .834 submit an inquiry to the Washington State Patrol to conduct a records check to verify the
answers provided above. You will be notified within (10) ten days after a response is received from the State
Patrol of the nature of the response and be provided a copy at your request. The District will use this information and record only to make the initial employment decision and for no other purpose.
